Why Does the Recommended Age for an Orthodontic Consultation Surprise So Many Parents?

Most parents assume braces are a middle school or high school concern, but the American Association of Orthodontists recommends children receive their first orthodontic evaluation by age 7. At this age, kids typically have enough permanent teeth coming in for an orthodontist to identify developing problems with jaw growth, bite alignment, and tooth spacing. An early evaluation doesn’t mean treatment is automatically needed, but it gives your orthodontist a clear picture of what’s ahead.

By age 7, the first molars have usually emerged, establishing the back bite. This makes it possible to evaluate front-to-back and side-to-side tooth relationships. Issues like crowding, crossbites, and protruding front teeth can often be identified at this stage, well before they become more involved to correct.

What Are Orthodontists Looking For in Young Children?

During a child’s first consultation, an orthodontist will evaluate much more than whether teeth are crooked. The assessment looks at the full picture of how the jaw, bite, and teeth are developing together. Some of the most common early findings include:

  • Crowding or spacing that suggests a jaw-to-tooth-size discrepancy
  • Crossbites, where upper and lower teeth don’t meet correctly
  • Underbites or overbites affect how the jaw closes
  • Early or late loss of baby teeth that could shift permanent teeth out of position
  • Thumb-sucking or tongue-thrusting habits impacting jaw development

Many of these concerns are far easier to address during active growth than after. That’s one of the primary reasons early evaluation exists, not to rush treatment, but to monitor and intervene at the moment it makes the most sense.

What Is Phase 1 Treatment, and Does My Child Need It?

Phase 1 early treatment is a type of interceptive orthodontic care designed for younger children, typically between ages 6 and 10, who have specific developmental concerns. Not every child who comes in at age 7 will need Phase 1 treatment. In fact, the majority won’t. However, for the children who do, early intervention can significantly reduce the scope of treatment needed later.

Phase 1 treatment might involve a palatal expander to address a narrow arch, partial braces to correct a serious crossbite, or other targeted appliances that guide jaw growth while the child is still developing. These are not the same as full orthodontic treatment, which typically begins once most permanent teeth have come in.

What Happens After Phase 1?

After completing Phase 1 treatment, most children enter a resting phase while the remaining permanent teeth emerge. This is followed by Phase 2 treatment, which addresses tooth alignment and bite refinement. Having Phase 1 behind them means Phase 2 is typically shorter and more straightforward.

How Do I Know If My Child Has Any Warning Signs?

You don’t have to wait until something looks obviously wrong to schedule a consultation. In many cases, early orthodontic concerns aren’t visible to parents at all. That said, there are some signs worth noting:

  • Difficulty chewing or biting
  • Mouth breathing, especially during sleep
  • Speech difficulties or persistent lisping
  • Teeth that appear to meet in an unusual way
  • Shifting of the jaw when the mouth opens or closes

Even if none of these apply, the age 7 guideline exists precisely because early evaluation catches what isn’t obvious yet. A consultation is non-invasive and gives you reliable information about your child’s development.

What Can Parents Expect at a First Orthodontic Visit?

The first visit is an evaluation, not a commitment to treatment.
