Dr. Lazzara earned his Doctor of Dental Surgery degree at the LSU School of Dentistry in 1996, and after completing dental school, he enrolled at the Center for Advanced Dental Education at St. Louis University. There, he completed a 30-month residency program in Orthodontics and earned his Master of Science degree.

Dr. Amy Bouvier Poblenz is originally from South Florida. She earned her DMD from Nova Southeastern University after graduating from the University of Florida with her BS. She completed a one year craniofacial research fellowship after dental school and continued on to earn her MS and specialty certificate in Orthodontics and Dentofacial Orthopedics at NSU. Prior to moving to Jacksonville in 2016, she practiced in the Nashville area for 3 years. Her husband, Charlie, is a general dentist practicing in San Marco. Together they have two young daughters, a dog, and a cat.

We know that you have a busy schedule, so we offer convenient office hours, including early morning and evening appointments, to fit your lifestyle. We also know that “life happens,” so if you need to cancel an appointment, please give us 24-hours’ notice – that way, we can rework appointments for our other patients and fit you in at a better time. If you miss a second appointment without 24 hours’ advance notice, you are subject to being charged an additional fee.
Our office is committed to helping you maximize your insurance benefits! Because insurance policies vary, we can only estimate your coverage in good faith, but cannot make guarantees due to all the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service.
As a service to you, we will bill your insurance company for services and allow them 45 days to render payment. After 60 days, please note that you are responsible for the entire balance, which is to be paid in full.
