Dear Patients, Families, and Friends:

We are happy to be back at work! Please take a moment to read this update, as we need to share some information about your next visit. 

Please fill out a AAOIC Supplemental Informed Consent form before your next visit. This online form only needs to be completed once for every patient.  

There are a few procedural changes that you will note at your next visit. None of them will require any extraneous effort on your part, but we want to be aware of these before your next appointment.

• When you get to the office, please stay in your car and call us at (904) 270-8750 to let us know you have arrived. A team member will greet you to check you in. We are taking this step to adhere to the social distancing protocols outlined in reopening guidelines. 
• Please brush your teeth before coming to your appointment. Our brushing station must remain closed at this time.
• We are asking patients to bring a mask to wear while in our office. Patients must wear masks at all times they are in the office. The only exception is during their actual appointment.
• A Wellness Screening will be performed upon arrival to take the patient’s temperature and answer a few health-related questions. We will include a link to the health questionnaire in your text appointment reminder. You can complete the form electronically before your visit. You must complete a screening questionnaire before each visit. You can also find the screening questionnaire here. 

Our prior standards for sterilization and cleanliness were already beyond that which have just been set forth by the CDC and ADA, but we are taking it up another level. We have added HEPA air filtration and are adding glass extensions to the existing partition walls in our clinics. Please don’t be alarmed by this added vigilance — we are simply being proactive.

If you have any more questions please ask when we call you in the coming week. We are grateful for your support and cooperation as we adjust to the “new normal.” We are so, so excited to have you back in the office, and we hope you’ll be just as excited to see us!
